If you are a first-time client, please complete the form below prior to our first session so not to take away from the optimal wellness experience possible; focusing on you and not crucial admin tasks related to your health & well-being.

We welcome you to fill out requested paperwork at the time of service, if needed, with the understanding that this time will be deducted from the remainder of the visit.
