"AGREEMENT OF SERVICES"

We ask that you please review our policies to help you have the best experience possible.

"Before You Visit & Cancellation Procedures"

~ A NEW CLIENT PERSONAL HEALTH INTAKE & AGREEMENT OF SERVICES and GENERAL LIABILITY RELEASE FORMS MUST BE FILLED OUT PRIOR TO YOUR FIRST SESSION.  This will confirm you have read, acknowledge, understand, and agree to comply with the professional policies & protocols listed below.  Your signed form will remain on file up for up to five years from your last session.

You can print these forms off at your convenience (located under the "Helpful Forms" link) and to help you save time for your initial session.  This helps keep you safe and gives your practitioner insight on how best to meet your needs & goals.  If these forms have not been completed before the first visit, time will be allotted from your session in order to do so.  We take joy in providing the best service & professionalism in the wellness industry and this is one of those things that ensures we keep you #1 and always in good hands!

~ The service(s) you are receiving are provided in a peaceful setting with a calm atmosphere; with that being said IT IS EXPECTED THAT: phones are silenced, common courtesy is given to those around you & if you bring a guest that you are responsible for their behavior. Please do not bring children unless they are receiving the service and are accompanied by a responsible guardian. If this policy is not followed; the service will be canceled and no refund or rescheduling of the service will be allowed due to negligence.

~ Massage Therapy is a valid form of healthcare(even if you are using it for "nonmedical purposes") and your therapist has set aside time for your appointment. WE DO TAKE PREPAYMENT AT THE TIME OF SCHEDULING; if proper cancellation procedures are followed- rebooking is permitted. NO REFUNDS ARE GIVEN due to client negligence of our cancellation/no shows/late arrivals policies.

~ CANCELLATION you agree to give at least 24 hours notice for cancellation

~ NO SHOWS you understand there are no refunds for missed session.

~ LATE ARRIVALS you agree to be on time and understand that if you are late to your appointment your therapist will deduct that time from the remaining scheduled session. Therapist will reach out after 5 min to see if ok and on your way and if you are late by more than 15 minutes or no contact has been made during that time; the session WILL BE CANCELLED & no refund will be given for missed session.

•IF YOUR THERAPIST CANCELS last minute due to unforeseen circumstances, illness, etc.- you will get your service RESCHEDULED along with a MATCHING SERVICE CREDIT equal to the initial service booked as we are all about HONORING ONE ANOTHER’S TIME.

~ You understand that a ‘session’ includes time for initial and post assessments, as well as allowing time for undressing and dressing (meaning actual table time can vary).

 ~ To allow Suruhåna Massage & Natural Healing Management and your therapist to abide by HIPAA privacy laws, you authorize us the right to leave phone messages regarding your appointments, mail documents, and/or contact you via email based on your personal contact preferences provided.  We will not share any of your information unless authorized by you (the patient/client) or unless compelled to do so by law.

~ Payment Policies when booking: CASH OR CREDIT CARD PREPAYMENT (in the amount of the service price), COUPON VOUCHERS & GIFT CERTIFICATES are acceptable forms of payment expected at the time of service being scheduled.  RESCHEDULING OF SERVICE(S) allowed with proper cancellation notice.

NO REFUNDS GIVEN; however exceptions may be made via SERVICE CREDIT at owner’s discretion based on circumstances 

*Flex Spending and Health Savings Account benefit cards are also welcome; i.e. HSA's & FSA's- with a prescription from your healthcareprofessional for therapeutic massage services 

~GIFT CERTIFICATES: must be redeemed within 5 years of purchase date (or as the law pertains in your state); a $30 annual deduction fee will be imposed for all gift certificates not redeemed within the 1st year of purchase date.  For "service" gift certificates purchased; rates are subject to change without notice- client acknowledges they must pay the rate difference if applicable.  GIFT CERTIFICATES ARE NOT REDEEMABLE FOR CASH.

~ Gratuity/Tip: is always appreciated but never expected** (customary is usually between 15-20% of the total service price).

** EXCEPTION: For SPECIAL EVENTS done by SMNH there is a 3 hour minimum, travel fee + 15% gratuity charge of the total service price**

~ Parking/Travel Fees: There is a $50 travel fee within 10 miles of our NW OKC location; after 10 miles fee to be determined.  As long as there is free parking for your therapist to provide your services; there will be no parking fee.  If your therapist has to pay for parking; client will be responsible for paying for that fee at the time (before) the service is rendered.

**CORPORATE/SPECIAL EVENTS have a $75 beginning travel fee within 25 miles of our office location

~ Sexual Misconduct and/or Aggressive Behavior etc.  is not tolerated.  Any of the behaviors listed below, but not limited to, will result in immediate termination of the session.  NO REFUNDS WILL BE GRANTED. If a session should terminate because of misconduct or inappropriate behavior, it will be the therapist's decision to involve law enforcement by calling 911 and your personal information will be provided to local law enforcement.

*THREATS OF PHYSICAL HARM

*VERBAL THREATS OR AGRESSION

*GESTURES, CRUDE LANGUAGE

*ANY ACT OF VIOLENCE 

•COVID-19/ILLNESS/INJURY/ACCIDENT PRACTICES•

~ Our staff & office is COVID COMPLIANT & we utilize proper sanitation procedures 

~ MASKS ARE OPTIONAL

~We ask that you do not schedule a session if you are experiencing any flu like or COVID symptoms 

or if you have been in contact with anyone that has in the last 14 days;

NORMAL CANCELLATION POLICIES APPLY for ANY AILMENT/ACCIDENT/INJURY; including COVID

(if needing to reschedule we require 24 hours notice unless a doctor's note is provided for last minute cancellation than a a SERVICE CREDIT OR REBOOKING OF SERVICE will be permitted)

~ We are always happy to provide you a copy of your invoice or any records necessary that you may need to submit to your insurance company or for your own personal records.

 ~ Our other services offered via phone or video consult still require being on time for your session and cancellation procedures met if necessary.
